PRISM™ System Deliverables:
- Patient Assessment Matrix (PAM) – disease risk tool training and guidelines
- Pain Health Assessment (PHA) – patient self assessment tool
- PRISM™ Summary Sheet specific to individual patients
- Access to raw data specific to individual patient response to PHA
- Clinical Performance Summaries regarding baseline patient information and patient response to treatment. Limited subsets of patients based on information captured in the PHA; such as chief complaint or physician.
- Comparative reports based on subsets of patients with other practices in the database.
- Web based training:
Initial training for physician - 2hrs
Initial training for staff - 2 hrs
Follow up training within 90 days - 1 hr - Clinical and IT support via e-mail unlimited for 1st 90 days
